Headache After a Rear-End Collision: Concussion, Whiplash or Cervicogenic Headache?

A headache that begins after a rear-end car accident can be confusing, especially when you never struck your head on the steering wheel, window, or headrest. The headache may start within minutes of the collision, appear later that evening, or become noticeable the next morning when neck stiffness begins to set in.

In some people, the problem is related mainly to the neck. In others, the rapid movement produced during the collision may contribute to a concussion. And sometimes the headache reflects a mixture of concussion symptoms, whiplash-associated neck pain, muscle irritation, and pain referred from the cervical spine.

This overlap is one reason a headache after a rear-end collision should not automatically be dismissed as “just whiplash.”

A concussion can occur after a force to the body causes the head and brain to move rapidly, meaning a person does not necessarily have to hit their head directly. Headache is also one of the common symptoms reported following concussion. Symptoms can appear immediately or may take hours or days to become noticeable. [1][2]

At the same time, sudden acceleration and deceleration of the neck can produce a whiplash injury, and headache is a recognized consequence of this type of trauma. International headache criteria specifically recognize both acute and persistent headaches attributed to whiplash. [3][4]

So how can you tell whether a headache after being rear-ended is coming from a concussion, whiplash, or the cervical spine?

The answer often lies not in the headache alone, but in what other symptoms accompany it and what makes the headache better or worse.

Why Can a Rear-End Collision Cause a Headache?

During many rear-end collisions, the torso is suddenly pushed forward with the vehicle while the head initially lags behind. The neck can rapidly move through extension and flexion as the forces of the impact travel through the body.

This type of acceleration-deceleration movement is consistent with the mechanism commonly described as whiplash. [3][4]

The effects of the collision, however, are not necessarily limited to the muscles of the neck.

A forceful hit to the body can cause rapid movement of the head and brain even without direct head impact. This is why concussion after a rear-end collision can occur without hitting your head. [1]

Meanwhile, joints, muscles, ligaments, and other structures within the cervical spine can become painful after trauma. Pain generated in the neck can sometimes be perceived in the head, producing what is commonly described as a cervicogenic or neck-related headache. [5]

Therefore, three different processes may be relevant after a rear-end crash:

  • headache associated with concussion or mild traumatic brain injury;
  • headache attributed to whiplash injury; and
  • headache generated by structures within the cervical spine.

Importantly, these categories can overlap.

Concussion Headache After a Rear-End Collision

Many people assume that a concussion requires losing consciousness or striking the skull against something. Neither is required.

A concussion is a mild traumatic brain injury that can occur from a bump or blow to the head or from a hit to the body that causes rapid movement of the head and brain. [1]

What Does a Concussion Headache Feel Like?

There is no single headache pattern that proves someone has sustained a concussion.

Post-traumatic headaches can resemble more familiar headache disorders. Research has found that migraine-like and tension-type headache patterns are common after traumatic brain injury. [6]

The more useful clue is often the presence of other neurological or concussion symptoms.

Someone with a concussion-related headache may also experience:

  • dizziness or balance problems;
  • nausea;
  • sensitivity to light;
  • sensitivity to noise;
  • blurred or altered vision;
  • fatigue;
  • difficulty concentrating;
  • problems remembering things;
  • feeling mentally slowed down;
  • feeling foggy or groggy;
  • irritability or emotional changes; or
  • changes in sleep. [2]

For example, a person who develops a headache after a car accident along with light sensitivity, dizziness, difficulty concentrating, and unusual fatigue may need to be evaluated for concussion rather than assuming the symptoms are purely muscular.

Can Concussion Symptoms Be Delayed After a Car Accident?

Yes.

Some concussion symptoms appear immediately, while others may become apparent hours or even days after the injury. [2]

This can happen when a person initially feels shaken but relatively normal after an accident and later notices symptoms such as headache, difficulty concentrating, dizziness, fatigue, irritability, or sleep disturbance.

The absence of an immediate headache therefore does not completely rule out concussion.

Whiplash Headache After Being Rear-Ended

Whiplash describes an acceleration-deceleration injury involving rapid movement of the head and neck. Although whiplash is commonly associated with rear-end collisions, it can occur in other circumstances involving similar forces.

A headache developing shortly after this type of neck injury is specifically recognized as a headache attributed to whiplash. [3][4]

Under international headache classification criteria, an acute headache attributed to whiplash develops within seven days of the injury and lasts for less than three months. When the headache continues for more than three months, it can meet criteria for persistent headache attributed to whiplash, assuming another diagnosis does not better explain it. [3][4]

What Does a Whiplash Headache Feel Like?

There is no single pain pattern that defines every whiplash headache.

The headache may occur along with:

  • neck pain;
  • neck stiffness;
  • pain at the base of the skull;
  • shoulder or upper-back discomfort;
  • reduced neck movement;
  • tenderness around the neck;
  • pain aggravated by certain positions or activities; or
  • headaches that seem to worsen as neck discomfort increases.

One important point is that “whiplash headache” describes the relationship between the headache and the injury more than a unique type of headache sensation.

The headache may resemble a tension-type headache, migraine-like headache, or headache generated predominantly by the cervical spine. [6]

What Is a Cervicogenic Headache After a Car Accident?

A cervicogenic headache is pain felt in the head but generated by a disorder or painful structure within the cervical spine or surrounding neck tissues. [5]

This concept is important after a rear-end accident because a person may complain primarily of headache even though the source of the pain is located lower down in the neck.

The cervical nerves and pain-processing pathways in the upper neck interact with pathways that carry sensation from the head and face. Because these signals can converge, the brain may perceive pain originating in the cervical region as headache.

Signs That a Headache May Be Coming From Your Neck

Certain features make a cervical source more likely.

These include:

  • headache that begins around the upper neck or back of the head;
  • pain spreading from the back of the head toward the forehead or eye;
  • headache triggered or aggravated by neck movement;
  • reduced cervical range of motion;
  • tenderness or pressure in the neck reproducing the headache;
  • headache associated closely with worsening neck pain; and
  • pain that consistently occurs more on one side.

Clinical criteria for cervicogenic headache specifically include reduced cervical range of motion with worsening of headache during provocative neck movements as evidence supporting a cervical source. [5]

Pain traveling from the back toward the front of the head and headache reproduced by head movement or pressure on certain neck structures can also support the possibility of cervicogenic headache, although these findings are not completely specific to the condition. [5]

Concussion vs. Whiplash vs. Cervicogenic Headache: What Are the Main Differences?

There is considerable overlap, so symptoms should not be used for self-diagnosis. Still, some patterns are helpful.

Concussion becomes more likely when headache occurs with:

  • dizziness;
  • light or noise sensitivity;
  • nausea;
  • blurred vision;
  • balance problems;
  • brain fog;
  • memory difficulties;
  • slowed thinking;
  • unusual fatigue; or
  • emotional and sleep changes. [2]

Whiplash-related headache becomes more likely when:

  • there was rapid acceleration-deceleration of the neck;
  • neck pain developed after the accident;
  • headache began soon after the collision; and
  • headache and neck symptoms appeared as part of the same injury.

Cervicogenic headache becomes more likely when:

  • neck pain and headache are closely connected;
  • turning or extending the neck worsens the headache;
  • cervical range of motion is restricted;
  • pressure on the neck reproduces familiar head pain; or
  • the pain appears to travel from the neck or back of the skull forward. [5]

But a patient does not always fit neatly into just one category.

Someone can sustain a concussion and cervical strain in the same accident. The resulting headache may therefore contain both neurological and neck-related components.

Can You Have Both Whiplash and a Concussion?

Yes.

The forces responsible for whiplash can affect the neck while rapid head movement can simultaneously produce forces capable of causing concussion.

That means someone may experience:

  • neck stiffness;
  • pain at the base of the skull;
  • limited neck movement;

along with:

  • dizziness;
  • nausea;
  • difficulty concentrating;
  • sensitivity to light;
  • fatigue; and
  • headache.

Trying to label every post-accident headache as exclusively “whiplash” or exclusively “concussion” can therefore oversimplify the problem.

A more useful clinical question is: Which systems appear to be contributing to the symptoms?

The cervical spine, vestibular system, visual system, headache pathways, and cognitive symptoms may all need to be considered when post-collision symptoms persist.

Why Does My Headache Start at the Base of My Skull After a Rear-End Collision?

Pain beginning where the neck meets the skull is common in people with neck-related headaches.

The upper cervical region contains joints, muscles, nerves, and other pain-sensitive structures. Irritation in this region can refer pain upward into the back, side, or front of the head.

A pattern in which pain begins posteriorly and travels toward the front of the head is one feature that can occur with cervicogenic headache. [5]

However, location alone does not establish the diagnosis.

Migraine-like post-traumatic headaches, occipital nerve irritation, muscle pain, and other disorders can sometimes produce pain in similar areas.

Is Headache the Day After a Rear-End Collision Normal?

Headache beginning the day after an accident is certainly possible.

Both concussion symptoms and musculoskeletal symptoms may become more noticeable after some time has passed. Concussion symptoms can emerge hours or days after injury, while neck stiffness and soreness may also become more apparent after the immediate stress of the collision has subsided. [2]

A delayed headache should therefore be assessed based on the complete clinical picture rather than dismissed simply because the person felt relatively well immediately after the crash.

The timing should also be documented accurately. International diagnostic criteria for trauma-related and whiplash-related headache use a headache onset within seven days of injury when classifying these conditions. [3][4]

When Should You Worry About a Headache After a Car Accident?

Most post-collision headaches are not caused by life-threatening brain injuries, but certain symptoms require urgent evaluation.

Seek emergency medical attention after head or body trauma if there is:

  • a headache that continues to worsen and does not go away;
  • repeated vomiting;
  • weakness or numbness;
  • decreased coordination;
  • a seizure;
  • slurred speech;
  • unusual behavior;
  • increasing confusion or agitation;
  • inability to recognize people or places;
  • one pupil becoming larger than the other;
  • loss of consciousness;
  • increasing drowsiness; or
  • difficulty waking the person. [2]

A rapidly changing neurological condition after an accident should not be watched at home simply to see whether it improves.

How Is a Headache After a Rear-End Accident Diagnosed?

There is no single test that can automatically distinguish concussion headache, whiplash headache, and cervicogenic headache in every patient.

Evaluation usually begins with the circumstances of the collision and the pattern of symptoms.

A clinician may ask:

  • Did the head strike anything?
  • Was there loss of consciousness?
  • Was there memory loss surrounding the accident?
  • When did the headache begin?
  • Where does the headache start?
  • Does neck movement worsen it?
  • Is there neck stiffness?
  • Is there dizziness or imbalance?
  • Is there sensitivity to light or noise?
  • Is concentration more difficult than usual?
  • Is there nausea or vomiting?
  • Are there visual symptoms?
  • Has the headache been improving or worsening?

A neurological examination may assess strength, sensation, coordination, balance, eye movements, mental status, memory, attention, and other neurological functions.

When a cervical source is suspected, examination may also include neck range of motion, tenderness, movement-related symptoms, and whether certain movements reproduce the person’s typical headache.

Do You Need a Brain Scan for Headache After a Rear-End Collision?

Not everyone with a post-accident headache needs imaging.

Concussion is largely a clinical diagnosis. A normal brain scan does not necessarily rule out concussion because routine computed tomography is intended primarily to detect structural problems such as bleeding rather than the physiological changes responsible for many concussion symptoms. [1]

Imaging becomes more important when the history or examination raises concern for structural injury.

For acute head trauma, factors such as worsening neurological findings, repeated vomiting, severe or progressive symptoms, or other clinical risk factors may influence the decision to obtain imaging. [7]

Depending on the circumstances, clinicians may use:

  • CT of the head;
  • CT of the cervical spine;
  • MRI of the brain;
  • MRI of the cervical spine; or
  • additional vascular imaging when a blood-vessel injury is suspected.

The appropriate study depends on what injury the clinician is trying to identify.

Can an X-Ray or MRI Scan Prove a Cervicogenic Headache?

Not necessarily.

Imaging can reveal problems within the cervical spine, but abnormalities seen on an imaging study do not automatically prove that those abnormalities are causing the headache.

This is particularly important because cervical imaging changes may also appear in people who do not have headaches. International diagnostic criteria therefore caution that imaging findings can support a diagnosis but are not, by themselves, firm evidence that a particular cervical abnormality is responsible for the pain. The relationship between the imaging findings, symptoms, physical examination, and clinical history matters.

How Are Headaches After a Rear-End Collision Treated?

Treatment depends on what appears to be driving the headache.

A person whose symptoms are primarily concussion-related may require a different recovery strategy than someone whose main problem is cervical joint and muscle dysfunction.

Treatment When Concussion Is Suspected

During the first few days after a mild traumatic brain injury, relative rest may be appropriate. Current recovery guidance generally favors gradually returning to light physical and normal daily activity after the initial short period of rest rather than prolonged complete inactivity, as long as activity does not significantly worsen symptoms. [8]

Patients should receive individualized advice about returning to:

  • work;
  • driving;
  • exercise;
  • sports;
  • screen-intensive activities; and
  • other activities with a risk of another head injury.

A second head injury before recovery can complicate the situation, so activities carrying a meaningful risk of another impact should be avoided until medically appropriate. [8]

Treatment When the Neck Is Contributing to the Headache

When cervical dysfunction is present, treatment may include appropriately selected rehabilitation focused on restoring movement and function.

Depending on the patient, this may involve:

  • cervical mobility exercises;
  • progressive strengthening;
  • posture and movement retraining;
  • endurance exercises;
  • graded return to normal activity; and
  • selected manual therapy.

Research evaluating cervicogenic headache supports a role for exercise and some forms of manual therapy, although treatment should be individualized rather than assuming every post-accident headache requires the same approach. [9][10]

Aggressive treatment immediately after significant trauma should not be undertaken before serious cervical injury has been excluded when the history or examination raises concern.

What If the Headache Lasts for Months After the Accident?

A headache lasting longer than expected deserves reassessment rather than simply continuing the same treatment indefinitely.

Under international classification criteria, a headache attributed to whiplash that continues for more than three months may be categorized as a persistent headache attributed to whiplash. [4]

Persistent post-traumatic headache can also follow traumatic brain injury. The headache may resemble migraine or tension-type headache, and treatment is often guided partly by the headache pattern and accompanying symptoms. [6]

Other factors may also keep headaches going, including:

  • ongoing cervical dysfunction;
  • sleep disturbance;
  • dizziness or vestibular symptoms;
  • migraine triggered by the injury;
  • medication overuse;
  • reduced physical activity;
  • persistent muscle tension; or
  • more than one of these problems occurring together.

In particular, frequent use of headache medication itself can contribute to persistent headache in some people, which is why medication use should be reviewed when symptoms become chronic. [4]

How Long Does a Headache Last After a Rear-End Collision?

There is no universal recovery timeline.

Many people with mild concussion improve considerably within the first few weeks, although symptoms can persist longer in some individuals. [2][8]

Whiplash-associated symptoms also vary widely. A mild cervical strain may improve relatively quickly, while more significant neck pain and headache can take longer.

Symptoms deserve further evaluation if they:

  • continue without meaningful improvement;
  • become more severe;
  • interfere with work or sleep;
  • repeatedly return with normal activity;
  • include persistent dizziness or neurological symptoms; or
  • develop new features that were not present earlier.

For concussion-related symptoms specifically, medical follow-up is recommended when symptoms do not go away within about two to three weeks or become worse after normal activities are resumed. [8]

Do Not Ignore the Neck When Evaluating Post-Concussion Headache

One of the challenges after a car accident is that dizziness, headache, balance problems, and visual complaints may immediately raise concern for concussion—and appropriately so.

But the cervical spine can also contribute to persistent symptoms.

Someone may recover from the initial neurological effects of concussion yet continue experiencing headaches because cervical dysfunction remains. Conversely, treating only the neck may fail when cognitive, vestibular, visual, or other concussion-related symptoms are still present.

This is why persistent headache after a rear-end collision sometimes requires broader assessment rather than repeatedly treating the headache itself.

What Should You Tell Your Doctor About a Headache After Being Rear-Ended?

The quality of the history can significantly help with diagnosis.

Try to describe:

  • exactly when the headache first appeared;
  • where it begins;
  • where the pain travels;
  • whether it is pressure-like, throbbing, sharp, aching, or burning;
  • whether neck movement changes the pain;
  • whether light or noise bothers you;
  • whether you feel dizzy or unsteady;
  • whether concentration or memory feels different;
  • whether you have nausea or visual changes;
  • how frequently the headache occurs;
  • how long each episode lasts; and
  • whether the overall pattern is improving, unchanged, or worsening.

It can also be useful to mention whether headaches existed before the accident and, if so, whether the headaches after the collision feel different in intensity, frequency, location, or associated symptoms.

The Bottom Line

A headache after a rear-end collision can come from the brain, the neck, or both.

Concussion becomes more concerning when headache is accompanied by symptoms such as dizziness, nausea, light or noise sensitivity, visual problems, difficulty concentrating, unusual fatigue, memory problems, or mental fog.

Whiplash-related headache is linked closely to the acceleration-deceleration injury itself and commonly occurs alongside neck pain and stiffness.

Cervicogenic headache is more specifically driven by the cervical spine and is often associated with restricted neck movement, headache provoked by neck motion, and pain that may travel from the back of the head toward the front.

The categories are not always mutually exclusive. A person who has been rear-ended may have cervical injury and concussion at the same time.

Most importantly, a worsening headache, repeated vomiting, neurological weakness or numbness, seizures, increasing confusion, abnormal drowsiness, unequal pupils, or difficulty waking after an accident requires urgent medical attention. [2]

When a headache persists after a collision, identifying what is actually generating the symptoms—rather than simply calling every post-accident headache “whiplash”—can help guide the next steps in evaluation and treatment.
